Job Summary
- Work as part of the Target Support team across designated Services to provide support and assistance to ensure Service User(s) needs are met through individual active support plans.
- Undertake all tasks and assist as required to ensure the smooth running of the Service to each individual using person centred approaches.
- Support people in the service with personal care as and when required respecting dignity, privacy and rights as far as possible promoting independence through active support principles.
Principle Accountabilities
- Provide Targeted Support to individuals.
- Supporting Service Users through day to day programmes, tasks and activities
- Attending and take an active role in regular person centred Reviews held by the Service and feedback on active support plan progress.
- Play a key role when Workshops/Away Days are organised.
- Work effectively through the course of each shift especially through shortages of staff.
- Be flexible in offering support to more Service User(s) in the Service.
- There is a requirement that medication training be undertaken for administration and recording safely
- Undertake sleep in duties as and when required.

Applicants must have at least six months experience of working within Supported Living, Residential or Day Service Provision. The successful post holder must be flexible and drivers are desirable and must hold a valid clean driving licence.
